Claims of Defence Safety Act Breaches Before Taipan Crash Surface

Documents tabled in the Senate following Senator Malcolm Roberts’s motion have revealed that the Department of Defence may have breached the Workplace Health and Safety Act in the lead-up to the fatal MRH‑90 Taipan helicopter crash in July 2023. The documents allege two major offences: reckless conduct and exposing workers to serious risk, citing known fatigue issues and concerns over the TopOwl 5.10 helmet-mounted sight display system. Despite these findings, Commonwealth prosecutors declined to charge Defence, prompting calls for transparency and accountability.
